Proposed Lucent HR Policy:

If the letter from the credentialed mental health specialist certifies
that the patient has been diagnosed with Gender Identity Disorder
(according to the current DSM) and that the patient has followed the
guidelines of the Standards of Care (according to the Harry Benjamin
International Gender Dysphoria Association) then we shall conclude
that the proposed treatment of Hormone Therapy and/or Sex Reassignment
Surgery is medically necessary and is covered by the plan.
